Our most popular question is, what insurance do we take? We specialize in auto accidents and work injuries, so we take almost all PIP and 3rd-party PIP claims, as well as WA state/self-insured workers' compensation claims. We are contracted with most health insurance plans, and yes, it's always a good idea to have a referral for coverage and authorization. For those who are not in-network or prefer our cash-rates, please give us a call at (425) 347-8614. If you have any questions in regards to insurance or coverage, please don't hesitate to reach out.

Please note: a provider may not be contracted with a specific insurance plan. Give us a call if you're unsure if we are in-network with your commercial insurance or have questions in regard to coverage, benefits, limits, etc.

1st Party PIP—we require claim information:● Claim number.● Claim adjuster.● Claim adjuster's phone number.
3rd Party PIP—we require claim information:● Claim number.● Claim adjuster.● Claim adjuster's phone number.● A letter of representation (LOR) and a letter of protection (LOP) from your attorney's office.***
***If the LOR & LOP cannot be provided, we may hold or discontinue care, otherwise you may be subject to the full cost of care.

We take WA state insured (L&I) claims. For private or self-insured workers' compensation, they must be licensed WA state TPAs (ie. Sedgwick, Corvel, CCSMI are common ones).
We require claim information:● Claim number.● Claim manager name.● Claim manager phone number.
All work injury claims are verified prior to your initial appointment(s) to make sure they are open & allowed.
